Before beginning the enrollment process, please be sure that you have the following documents ready to upload to the application. Once you begin, the application will not save or complete without these documents uploaded.

• Mortgage, lease agreement, or Property tax statement of residential dwelling (if the mortgage, lease agreement, or tax statement does not have the parent/ legal guardian's name, please visit the school to get a residency affidavit. It will need to be notarized and returned to Ms. Evans ~the affidavit has to be completed yearly if needed)

• Current Utility Bill (power or water bill in parent/ legal guardian's name)

• Copy of license or state issued ID of parent/ legal guardian

• Certified Birth Certificate of student (not hospital copy)

• Immunization record of the student
